"It is with enormous sadness that we report a fatal accident that occurred on November 29, during the preset of the show Luzia in San Francisco", a statement issued by Cirque du Soleil reads.

A Cirque du Soleil technician died from his injuries after he was hit by a telescopic lift Tuesday during a setup for a show in San Francisco. "Out of respect, the IATSE will be cancelling its plans to leaflet the Cirque du Soleil's OVO show at the Bell Centre on Dec 1", the union said in a statement on Thursday. "Olivier has always been a member of our tight family and a truly beloved colleague".

The Tuesday and Wednesday performances in San Francisco were canceled.

AT & T Park is located in the South Beach area, very close to downtown San Francisco. When they arrived, they found a male employee of the show suffering from "traumatic, life-threatening injuries", she says.

The incident has reportedly reignited concerns over safety at Cirque du Soleil performances, according to CNN.

Daniel Lamarre, the circus' CEO, issued a statement today saying he was "heartbroken" by the accident.

"It is with vast sadness that we report a fatal accident that occurred on November 29, during the preset of the show Luzia in San Francisco", the statement reads. The Chronicle notes that SFPD and Cal-OSHA investigators were both on the scene last night at Luzia, whose tent has been set up in the ballpark's Parking Lot A.

Earlier this week, a former Olympic gymnast fell face first during a performance with the company in Brisbane, according to 7 News, a CNN affiliate in Australia. In November 2013, the company was proposed fines for more than $25,000 for safety violations and for the death of French performer Sarah Guillot-Guyard, 31.
